Step 2 of 3: Make Appointments
Make an appointment for a risk screening visit with your obstetrician and another for your first prenatal with your midwife.
To your midwifery prenatal, please bring your paperwork. If you have already seen your OB, bring your completed OB risk screening form, the prenatal notes, and the lab reports from your OB visit.

You may already have these forms in your consult packet. If not, you can print extra copies here. Please bring these completed forms to your first midwifery prenatal.
